Community Case Management Services Ltd (CCMS) is a specialist provider of case management for the rehabilitation of severely injured adults and children throughout the United Kingdom, Europe and Worldwide. CCMS provides quality-of-life solutions for a wide range of clients who have suffered catastrophic / complex and life-changing injuries such as acquired brain injury, spinal cord injury or loss of limb. At CCMS, we ensure that the highest level and quality of care and support is provided and tailored to individual needs. CCMS case managers support our clients who have a high level of complex clinical and rehabilitation needs and ensure that they are encouraged and given the opportunity to live their lives to the full.
CCMS have an experienced and skilled team of clinical case managers and assistant case managers many of whom have worked with CCMS for more than 5 years.

Several of our case managers speak more than one language, and are familiar with the health, care and rehabilitation services and regulatory compliance in other countries, including but not limited to Romania, France, Greece, Turkey, Jersey, and Guernsey.

Our clinical case managers are qualified professionals and hold the required professional memberships with RCN, RCOT, CSP, SWE, RCSLT, BPS.They are members of NMC and HCPC, and hold memberships with BABICM, and / or CMSUK. Our case managers and assistant case managers all hold an Enhanced DBS with Barring for both the Adult and Children’s Workforces.

CCMS case managers and assistant case managers participate in on our Case Management Pathway Programme which runs throughout the year to ensure a full understanding of the litigation process, as well as undertaking their own continuing CPD.
