Empowering U Case Management (EUCM) was born out of a requirement to build a Case Management firm that delivers a client-first, collaborative approach to case management. By sitting under the wider Empowering U umbrella, we have access to a vast amount of expertise and experience gained in the health and social care sector and use this to deliver a unique service, putting the client at the heart of everything we do.

Our approach to Case Management is purely centred around the individual, we listen to them and make sure they are part of every conversation and every decision. Our relationship with the client starts before a Case Manager is even assigned, and we work together to agree who is the most appropriate fit for them.

Following a free meet and greet with their chosen Case Manager, we start the rehabilitation process together, all whilst keeping cost in mind.

At Empowering U Case Management, our Case Management service specialises in offering clients every available opportunity to re-enable and lead a fulfilling life after such event.

We work collaboratively with the client to set person centred measurable goals as part of their rehabilitation. We build resilience and confidence and support safe positive risk taking. We support our clients to optimise recovery and take control of their lives.

We ensure our Case Managers have the appropriate skill set as well as experience of working within a multi-disciplinary team and the ability to advocate for a client. To underpin all this, they must follow our client-centred approach. Through our comprehensive induction and ongoing support and governance we ensure our case managers are best placed to help our clients optimise their recovery.

Empowering U Case Management and the team ensure they follow the Rehabilitation Code (2015) and align to BABICM, CMSUK and VRA guidelines. We also keep updated with the development of the Institute of Registered Case Managers (IRCM).
