Brain Injury Rehabilitation Success Story: Ryan’s Journey to Recovery

When Ryan arrived at Active Care Group’s Nottingham Brain Injury Rehabilitation Centre, his situation was critical. He had sustained a Traumatic Brain Injury (TBI) after an unwitnessed fall from a full flight of stairs. The injuries were severe: traumatic encephalomalaciaa subdural haematoma, and multiple facial and skull fractures. Alongside these, an ingested foreign object required monitoring, and his physical condition was significantly compromised.

At the time of admission, Ryan could not lift his head. He had no control over his saliva and was entirely dependent on others for mobility and care. His nutrition was delivered through a PEG (Percutaneous Endoscopic Gastrostomy) tube, and even the simplest tasks required full verbal and physical assistance. This wasn’t just a significant injury but the beginning of a long and uncertain journey toward recovery.

The Initial Challenges of Traumatic Brain Injury

Ryan’s admission marked the beginning of a demanding rehabilitation journey. As a result of his severe brain injury, he faced complex, overlapping challenges that affected both his body and mind. Balance, strength, and stamina were all profoundly reduced. He could walk no more than a few metres and required an attendant-propelled wheelchair for any movement beyond that.

Cognitively, Ryan needed constant prompting to engage in everyday tasks. Activities like washing, dressing, and managing basic hygiene had to be guided by both verbal instructions and hands-on assistance. His physical limitations, combined with the psychological toll of his injury and his pre-existing mental health history, meant that progress would require consistent, structured support.

Cognitive functioning

Required cues and supervision for all self-care tasks

Feeding and hydration

Dependent on a PEG tube for all nutrition

Emotional and behavioural

Pre-existing mental health issues, history of drug use

Lack of independence

Fully reliant on the care team for daily routines

Our Rehabilitation Approach: Personalised and Persistent

At Brain Injury Group, we understand that every person’s recovery is different, and Ryan’s path required a personalised, phased approach. From the outset, the focus was on helping him rebuild basic functions—starting with head control, swallowing, and short-distance mobility. Ryan’s allocated Multi-Disciplinary Team (MDT), including Physiotherapists, Occupational Therapists, and Nursing Staff, worked together to create a consistent and achievable rehabilitation plan.

Ryan’s progress was measured in small but meaningful steps. Over time, he regained the ability to move short distances unaided and showed increased stamina. The Speech and Language Team Active Care Group played a vital role in restoring his ability to eat and drink independently, helping him transition safely from PEG feeding to oral intake. This was a significant milestone, both medically and psychologically.

Overcoming Barriers: From PEG Tube to Independent Living

One of Ryan’s most significant turning points was his transition from PEG feeding to independent oral intake. This shift marked progress in his physical recovery and restored a sense of normalcy and dignity. His Speech and Language Therapy Team conducted thorough assessments and supervised a step-by-step reintroduction of food and fluids, ensuring safety throughout the process.

As Ryan’s strength and coordination improved, he regained independence in daily activities. With ongoing support from occupational therapy, he gradually learned to manage personal care, prepare basic meals, and move confidently indoors and outdoors. Perhaps most remarkably, by the time he was ready for discharge, Ryan was no longer reliant on physical assistance for these tasks.

Rebuilding Life Beyond Recovery

Once Ryan had regained physical independence, the next phase of rehabilitation focused on reintegration into his home, community, and daily life. To ease this transition, a graded discharge plan was introduced. It started with brief home visits and progressed to overnight stays and weekends away. This gradual exposure helped Ryan build confidence and adjust to life outside the centre.

Despite relocating to a different country, coordination between our team and local services ensured Ryan had the right support. Although he did not require a formal care package, Active Care Group arranged contact with addiction support providers in his hometown. Ryan agreed to regular drug and alcohol testing, and he remained substance-free throughout his time with us.

Specialist Support from Active Care Group

Ryan’s transformation would not have been possible without the expert support of Active Care Group, whose facilities, including the Nottingham Brain Injury Rehabilitation Centre,  are designed to meet the needs of individuals with complex neurological conditions. Their focus on individualised care pathways, round-the-clock clinical oversight, and multi-disciplinary rehabilitation made a significant impact on Ryan’s ability to recover physically, cognitively, and emotionally.
